تشخیص عفونت هلیکوباکتر پیلوری با آزمایش‌های تهاجمی و غیرتهاجمی

چکیده

تشخیص عفونت هلیکوباکتر پیلوری با روش‌های مختلف تهاجمی و غیرتهاجمی امکان‌پذیر است، اما هر روش محدودیت‌های خاص خود را دارد. در این مطالعه، ۸۹ بیمار (۶۱ بزرگسال، ۲۸ کودک) که برای اندوسکوپی بالای دستگاه گوارش به بیمارستان فیروزگر و مرکز پزشکی کودکان مراجعه کرده بودند، وارد مطالعه شدند. آزمایش‌های غیرتهاجمی شامل بررسی آنتی‌بادی‌های سرولوژیک و شناسایی آنتی‌ژن هلیکوباکتر پیلوری در مدفوع انجام شد و بیوپسی‌ها برای بررسی هیستولوژی، تست اوره‌آز سریع (RUT) و PCR استفاده شدند. نتایج PCR نشان داد که ۵۳ بیمار مثبت بودند. هیستولوژی حساسیت و ویژگی ۱۰۰٪ و ۹۰٪ داشت و عملکرد بالایی در بزرگسالان و کودکان نشان داد. حساسیت، ویژگی و دقت تست آنتی‌ژن مدفوع به ترتیب ۸۷٫۸٪، ۷۵٪ و ۸۲٪ بود. همبستگی RUT، سرولوژی (IgG)، هیستولوژی و تست آنتی‌ژن مدفوع با PCR به ترتیب ۰٫۸۲، ۰٫۳۲، ۰٫۹۱ و ۰٫۶۳ بود. نتایج نشان می‌دهد که RUT و هیستولوژی به دقت PCR بیوپسی هستند و تست آنتی‌ژن مدفوع می‌تواند به عنوان یک روش غیرتهاجمی مناسب برای تشخیص عفونت هلیکوباکتر پیلوری استفاده شود.

Abstract

Diagnosis of Helicobacter pylori infection can be performed using various invasive and noninvasive tests, each with its own limitations. In this study, 89 patients (61 adults, 28 children) referred for upper gastrointestinal endoscopy at Firoozgar Hospital and Children Medical Center were enrolled. Noninvasive tests included serological antibody assays and fecal antigen detection, while biopsies were used for histology, rapid urease test (RUT), and PCR. PCR results identified 53 positive cases. Histopathology demonstrated high performance in adults and children with sensitivity and specificity of 100% and 90%, respectively. Sensitivity, specificity, and accuracy of the stool antigen test were 87.8%, 75%, and 82%, respectively. Correlation of RUT, serology (IgG), histopathology, and stool antigen tests with PCR were 0.82, 0.32, 0.91, and 0.63, respectively. The study concludes that RUT and histopathology are as accurate as biopsy PCR, and stool antigen test can be considered an appropriate noninvasive method for detecting Helicobacter pylori infection.
